Medical Records

For your convenience we have provided an online copy of our medical release form.  No fee is required if records are sent directly from physician to physician.  If you would like for us to release information directly to you there is a $25.00 charge for the first 20 pages and a $0.15 charge for each additional page thereafter. 

If you do not have access to a printer please call our office and a medical release form can be faxed or mailed to you.
