Self Pay

If you are uninsured and are needing urological care, we can see you on a self pay basis. Payment will be due at the time services are rendered with a $300.00 deposit collected prior to seeing the physician. Upon checkout any amount owed to you will be reimbursed at that time. If a remaining amount is due for services provided, the balance will be due upon checkout.
